99.9% of your patients are not in your facility.

Which ones are in trouble?

Which ones are being admitted?

Which ones are being discharged home?

Which ones missed their dialysis appointment?

Which ones are being seen in an emergency department?

Which ones are being transitioned to a skilled nursing or outpatient rehab facility?

Position Health’s real-time notifications and longitudinal data when your patients are entering and exiting medical facilities enable provider teams to intervene when decisions are being made about a patient’s care.

Patient Population

Heavy utilizers

  • These individuals, due to complex medical problems, extensive co-morbidities, and challenging social circumstances, are high utilizers of acute care resources and are constantly transitioning into acute and subacute settings.

  • During these transitions, longitudinal providers - including PCPs, specialists, and case managers - are often unaware and therefore have no opportunity to impact the patient’s care.

  • Further, lack of awareness of care transitions out of acute and subacute settings makes it challenging to arrange for intensive wrap around services that will help keep the patient on the right trajectory.

Healthy individuals presenting at the emergency department

  • Emergency department providers with no knowledge of a patient often take a conservative approach, which leads to a full workup being performed for low risk patients.

Office Visit Benefits

At-risk appointment detection

  • Providers are notified in advance when a patient is likely to miss an upcoming appointment.

Scheduled appointment adherence tracking

  • Longitudinal providers can learn about missed appointments for their patients in real-time.

Emergency Department Benefits

Admission reduction

  • Timely intervention by longitudinal providers can avoid hospital admissions.

  • Unnecessary admissions are costly and detrimental to patients.

Wasteful care reduction

  • Duplicative diagnostic testing is costly and has negative health implications for patient.

Suboptimal care reduction

  • Longitudinal providers have internal knowledge about what works for specific patients that emergency department providers do not have.

Leakage reduction

  • When patients leak to out of network facilities and providers, there are no negotiated rates in place.

  • Rate negotiation for these cases is costly and rate management capabilities are diminished.

  • For patients, care is less consistent when delivered outside network.

Safe home transitions

  • Notification to longitudinal providers when patient transitions home increases chance that patient stays on the right trajectory.

Hospital Admission Benefits

Safe home transitions

  • Notification to longitudinal providers when patient transitions home increases chance that patient stays on the right trajectory.

Skilled Nursing, Inpatient Rehab Admission Benefits

Safe home transitions

  • Notification to longitudinal providers when patient transitions home increases chance that patient stays on the right trajectory.

Length of stay monitoring

  • Insurers and longitudinal providers can use real-time length of stay data to make sure patients are kept in facility only as long as required.

User Privacy

We designed and built our system with a focus on user privacy. Due to our use of geofencing, we do not track the location of our users. In fact, we only know where our users are when they enter one of our geofences.

The Position Health SDK collects the following information

  • When a user enters or exits a medical facility in the United States.

  • Which medical facility they entered or exited.

  • Time of entry or exit.
